An employee was standing on the fourth or fifth rung from the top of a 10-foot A-frame ladder, using an impact drill to remove cottonwood debris from louver screens. He fell to the concrete floor below and suffered injuries to the right side of his body: a wrist fracture, broken ribs, a pelvic fracture, and a partially collapsed lung.

An employee was loading a box truck. He was sitting on a stack of 20 doors (each door was 1 inch thick) that had been wrapped and stacked onto a 4-inch-high pallet and was in the bed of the truck. He then fell off the stack of doors and onto the truck bed (a distance of about 24 inches), fracturing his left femur and left wrist.

An employee was operating a Haeger Press and inserting a pem stud into a flat steel part. While his hands were between the tooling on the machine, the machine activated causing his left thumb to be caught between the top anvil and the steel part and amputating the tip.

An employee was unloading a tractor trailer using a reach powered industrial truck (PIT). The tractor trailer pulled away from the dock while the employee was exiting the trailer on the PIT, causing the PIT to fall between the trailer and the loading dock. The employee suffered a broken left clavicle and facial fractures.
